Criminal Background Check and Urine Drug Screen

Each student must have a criminal background check to verify identity and any past criminal history.  This must be completed before school begins and will be repeated before you begin the Clerkship portion of your DO program.

To order your criminal background check and urine drug screen:

  1. Go to and select "Place Order."
  2. Select package code MI37: Background Check – Urine Drug Screen
  3. You will then be directed to set up your CertifiedProfile account.  Required personal information includes:  full name and date of birth, social security number, current address, phone number and email address.
  4. Select method of payment ($90.75)
  5. Please note that international students will be required to complete an additional international background check via an additional order code, which may incur an additional fee.  For Canadian students, this background check code is MI37CA.
  6. The results of your criminal background check and urine drug screen will be sent directly to MSUCOM.


All students must be up-to-date with vaccinations and titers as determined by the University Physician throughout their tenure at the college.

Students that fail to become compliant by the deadlines will be suspended from participation as a College of Osteopathic Medicine student until such time as the student becomes compliant.

This includes processing the student’s withdrawal from the college, dropping all current semester enrollment and future semester enrollment. This could negatively impact financial aid disbursement and could require repayment of aid received for the current semester. Additionally, a letter of unprofessional behavior will be placed in the student’s academic file with reference made to it within the student’s Medical Student Performance Evaluation.
